JOB #1061  RN HOSPITAL LIAISON

RN and KY licensure required.  Minimum of two years experience in a medical/surgical/ ICU setting as an RN.  One year home health experience and one year management experience preferred.  Knowledge and ability to provide comprehensive nursing case management.  Ability to coordinate client services.  Must have current CPR certification.

Applicant must possess excellent communication and organizational, and time management skills.  Position requires travel to Lexington hospitals and facilities.  Liaison will obtain information for coordination of Home Health services and work closely with Community Educators and Clinical Directors on patient needs.

• Develop and maintain contact with healthcare professionals (e.g., physicians, discharge planners, social workers, healthcare coordinators) to provide updated information on NR home health program offerings, guidelines, and policies.
• Conduct pre-discharge hospital visits at the physicians’ request to determine need and eligibility for homecare.
• Coordinate discharges for discharge planner and social workers in defined facilities.

JOB #1057 LYMPHEDEMA CERTIFIED THERAPIST

Licensure from a physical or occupational therapy curriculum approved by the APTA and/or the Council on Medical Education of the American Medical Association.  Minimum of one year PT/OT experience required with ability to provide comprehensive evaluation and treatment.   Must be trained in the four components of Complete Decongestive Therapy.  The  therapist will be able to differentiate between a variety of peripheral edemas as well as understand the applications, medical indications, and contraindications of MLD and CDT. Additionally, the therapist will be able to establish a CDT treatment plan for individuals with primary or secondary lymphedema and perform the indicated treatment.  Knowledge of the team concept and ability to function within this concept.

Licensure from an occupational therapy curriculum approved by the AOTA and/or the Council on Medical Education of the American Medical Association. Minimum of 1 year OT experience required.
