COMMUNITY EDUCATION COORDINATOR
College degree required.
Background in nursing, social services, public
relations, and/or relevant medical field. Work
experience in medical human services, health field,
clinical products and/or services environment.
Knowledge of the team concept and ability to function
within this concept. Ability to promote and maintain
positive interpersonal relationships utilizing highly
effective communication skills. Ability to identify
home care needs of patient, community populations and
resources to meet more of their needs. Effective time
management, self motivation and organizational skills a
must. Duties include, but are not limited to:

Promote care coordination
for physicians and rural hospitals for current patients.
·
Coordination with
physicians to ensure appropriate and accurate written
documentation (orders, case summaries, etc.) is obtained
and returned to the agency in a timely manner.

Obtain referral information
from physician offices after appropriate referral for
care is received.
·
Education to community and
hospital staff on appropriate resources available for
home health.
·
Education activities to
inform community and physicians regarding qualifying
criteria for home care.
